All Specimens must have the following:

• Information
  • Patients Name and Address
  • Date of Birth
  • Hospital Name and Address
  • Insurance information (Hospital admission form)
  • Pathology Accession Number
  • Requesting Physican Name, Address and Phone
• Billing
  • Institutional billing
  • Patient Insurance billing (must include patient insurance billing information)

Transportation of specimen to UTHSCSA histopathology laboratory

The biopsy specimen should be shipped as soon as possible to the:
