Sending the Sample

Specimens (See Specimen Requirements) need to be sent protected from heat to:

Please include, along with the specimen, a completed Flow Requisition Form containing:

1. Physician’s name, phone number and preferred mailing address.
2. Hospital information (i.e. patient’s ID number, sex, date of birth etc.)
3. Provisional diagnosis
4. Any pertinent patient history.
5. Billing information

Please call the lab weekdays 8:00 a.m. to 5:00 p.m. at 210-450-8345

Or call Client Services after hours or on weekends at 210-567-6599 with any questions.

NOTE: For prompt processing, the specimen should be received by 4:00 p.m. on weekdays.
